From the 1st October 2022, Breightmet & Little Lever patients can now access more GP, Practice Nurse and Healthcare Assistant appointments in the evening or at the weekend from our local Extended Access Primary Care site at Tonge Fold Health Centre.
Breightmet & Little Lever PCN is committed to improving access to primary care services. This includes making appointments available at a time that is convenient to patients.
If you:
- find it difficult to attend an appointment during the working day;
- are a busy parent with small children;
- or rely on working carers to take you to appointments;
then this service is for you.
About this Service
Breightmet & Little Lever PCN has arranged for additional appointments to be provided outside of normal GP practice hours. The service offers pre-bookable and same-day routine primary care appointments with a range of clinicians, including GPs, Practice Nurses and Healthcare Assistants.
Please be aware that this is not a walk-in service – you must have an appointment.
Appointments
An appointment must have been booked in advance of visiting the service.
To book an appointment for this service, please contact the practice on 01204 463777. When you book an appointment, the practice will ask you for your consent to share your detailed GP record with the clinician providing the care.
Appointments are available:-
Monday to Friday 18:30 – 21:30
Saturday 09:00 – 17:00
If you already have an appointment booked and need to cancel this, please contact the practice on 01204 463777 08:00 – 18:30 Monday to Friday. If you need to cancel an appointment at this service and we are already closed, you can ring the service directly on 01204 462060. Please note, the phone line for this service is only open when the Extended Primary Care Service is open (see times above). You cannot book an appointment by ringing this telephone number, you must make an appointment by ringing our practice.

This service does not cover emergency conditions that require immediate direction to A&E, such as chest pain and severe shortness of breath, pregnancy related emergencies, dental emergencies or mental health emergencies.
